MDSC Annual Awards Nomination Form

  • Each year, during the luncheon portion of our Annual Conference, MDSC honors individuals or organizations who have celebrated the rich accomplishments of people with Down syndrome. We invite members of our community or the general public to nominate those people whom you believe are most deserving in the following categories: Allen C. Crocker Award of Excellence (MDSC’s highest honor), Outstanding Educator Award, Exemplary Employer Award and Media Award.
  • The Allen C. Crocker Award of Excellence is given annually at the MDSC conference in recognition of Dr. Allen Crocker’s many years of nurturing individuals with Down syndrome and their families throughout their life’s journey. The award is presented to a person or group that celebrates the excellence of persons with Down syndrome and significantly advances the Down syndrome movement. It is the highest honor given by the MDSC. Nominating letters should include specific examples rather than general statements about the person.
  • The Media Award is given to a person or company who exemplifies the MDSC mission to disseminate up-to-date information about Down syndrome. Nominating letters should include specific examples rather than general statements about the person or company. Describe the advertisement, media message, news article, or television program. What positive message about Down syndrome did the media convey?
  • The Outstanding Educator Award is presented to an educator in Massachusetts who has significantly contributed to ensuring that students with Down syndrome are given every opportunity to maximize their full potential. Nomination letters should include specific examples, including: How does this educator maximize the full potential of students with Down syndrome? How long has this educator worked with students with Down syndrome? In what capacity has this educator worked with students with Down syndrome? What makes this educator unique? What motivates this educator? The MDSC is particularly interested in honoring educators who have made a difference in the lives of multiple students with Down syndrome. The most successful nominees are often those who have touched many lives and implemented systemic changes to their school system. (You may want to include names and contact information for other parents who would be willing to comment on this educator's impact.)
  • The Employer Award is given to a person or company who recognizes the inherent value that people with Down syndrome can bring to the workforce and act as role models for other employers to emulate. Nominating letters may answer the following: What is the employer's record of hiring people with Down syndrome and/or other disabilities? Give an example of at least one employee with disabilities for works for the employer. How has the employer helped create an inclusive work environment? How has the hiring of people with disabilities been good for the employer, employee, co-workers and/or customers?
